INSPECTION RECORDS GUIDE

An inspection audit trail should explain the decision from question to closure.

Learn what evidence must be retained so a later reviewer can understand what was checked, what failed, who acted and why the record closed.

THE SHORT ANSWER

An inspection audit trail is the chronological, attributable record of the inspection standard, responses, evidence, findings, status changes, corrective work and verification decisions.

What the audit trail contains

Retain equipment identity, template and version, inspector, timestamps, answers, evidence and contextual comments. For changes, capture the previous value, new value, actor and reason where appropriate.

A PDF of final answers alone may not explain reassignment, deferral or the control applied while work remained open.

From finding to verification

Link each finding to its risk, immediate control, accountable action, due date, completion evidence and verification. Preserve relationships if corrective work is performed through a separate work order.

Do not erase the original non-compliance when it is corrected; the before-and-after history is the evidence.

Audit-trail quality

Use synchronised timestamps, authenticated identities, appropriate permissions and immutable or append-oriented history for significant events. Define retention and export arrangements.

An audit trail is valuable only when records are intelligible. Consistent terminology and clear asset identity matter as much as technical logging.

COMMON QUESTIONS

Inspection audit trails explained FAQs

Is a timestamp alone an audit trail?

No. A useful audit trail also identifies the event, actor, affected record and relevant before-and-after context.

Should corrected findings remain visible?

Yes. Retain the original result and subsequent correction and verification rather than replacing history.

Can audit trails be exported?

A system should provide readable export or retrieval appropriate to customer, regulator and continuity needs.
